Certificates needed for legal purposes are $30 (includes HST). Please fill out a baptismal or marriage certificate request form.
This form should only be used for legally eligible requests for certificates from Ontario United Church congregations within the boundaries of Canadian Shield Regional Council, Antler River Watershed Regional Council, Western Ontario Waterways Regional Council, Horseshoe Falls Regional Council, Shining Waters Regional Council, and East Central Ontario Regional Council, (formerly Manitou, Bay of Quinte, London, Hamilton, and Toronto Conferences). For requests for certificates from other regions of Canada please consult the appropriate Regional Council Archives.
The certificate form should be completed only after a search of our holdings in the Archives catalogue has indicated that the baptism or marriage records are at the Archives. The form is not meant for family history research.
