Historic Marriage Certificate Form

Please use this form for requests for marriage records that date before 1900. If research is required to locate the record, an Archives staff member will contact you with more information on this process.

Please note that the wait time is currently indefinite as we work through the large influx of requests.

A fee of $50 (HST included) is required for each pre-1900 marriage certificate. The Archives is currently issuing these certified transcripts electronically. If multiple physical copies are preferred, please indicate how many paper copies you are wishing to purchase.

This form should only be used for certificates of marriage from Ontario United Church congregations within the boundaries of  Canadian Shield Regional CouncilAntler River Watershed Regional CouncilWestern Ontario Waterways Regional CouncilHorseshoe Falls Regional CouncilShining Waters Regional Council, and East Central Ontario Regional Council, (formerly Manitou, Bay of Quinte, London, Hamilton, and Toronto Conferences). For requests for certificates from other regions of Canada please consult the appropriate Regional Council Archives.
