Baptismal Certificate Form – United Church of Canada Archives

A fee of $30.00 (includes HST) is required for each baptismal certificate. The requester will be invoiced by the United Church of Canada Archives and will be provided with instructions regarding payment methods.

Please note: Effective May 8, 2026, certified copies of baptismal, marriage, and burial records from before 1900 are subject to a fee of $50 (HST included). Fees for certificates dated 1900 or after will remain at $30 (HST included).

This form should only be used for legally eligible requests for certificates of baptismal from Ontario United Church congregations within the boundaries of Canadian Shield Regional Council, Antler River Watershed Regional Council, Western Ontario Waterways Regional Council, Horseshoe Falls Regional Council, Shining Waters Regional Council, and East Central Ontario Regional Council, (formerly Manitou, Bay of Quinte, London, Hamilton, and Toronto Conferences). For requests for certificates from other regions of Canada please consult the appropriate Regional Council Archives.
